Every automated check passed, because the empty picture slot still rendered as a clean styled box. A human looked at the live page and saw three big blank squares. The rule we wrote that night: when you reuse a design, list every slot it has. Fill each one or cut it. A styled empty box is not a finished card.
The Narration Sounded Smart. The Gate Said Too Smart.Our reading-level check flagged a batch of course narration. The sentences were long, with three ideas packed into one line. Listeners would have to work to follow it. We split the long sentences and swapped the fancy words for everyday ones. The ideas did not change. The work got easier to follow. If your writing needs a second read to understand, it failed the first read.
A Reviewer Snuck an Invisible Character Past Our FilterWe have a filter that blocks certain words from ever being published. A reviewer tested it by hiding an invisible character inside a blocked word. The filter read it as a different word and let it through. So we changed the filter: now any invisible character is itself a violation, because normal writing never needs one. A good test does not ask if your guard works. It asks how to get past it.
